To be considered for a dealer account, please complete the Dealer Application below and submit the following documents together by email to customerservice@skil-care.com:

• Dealer Application
• Sales Tax Resale Certificate (if applicable)

For your convenience, the required forms are provided below and can be completed digitally. Simply save the files to your computer, complete them, and email the completed documents as attachments.

Once we have received and reviewed your application package, one of our authorized representatives will contact you regarding the status of your application and the next steps in establishing your dealer account.
